Same as in English, you make it clear through emphasis:
*I* didn't believe that "wheel" is {rutlh}... But how did *YOU* know that?
(...that it's wheel)
or
I didn't believe that "wheel" is {rutlh}... But *how* did you know that?
(...that I didn't know)
