{Qagh} is also a noun: "mistake." "I find one mistake here."

The verb suffix {-Ha'} has the meaning of having done something wrong or
opposite to the manner in which it is usually performed. Thus, {ghItlhHa'}
is "mis-wrote." ghunchu'wI' was right: I did mean to use the word {qablIj}.

Hmmm . . . I meant to say "Why does one always err when one is reporting
errors?" I suppose the problem comes in with the word {ja'}. We don't know
exactly how to use it. I think my life's mission will be to track down Marc
Okrand, and hold a {betleH} to his throat until he explains exactly how to
use the various words of speaking!
