Though I don't remember any canon use like this, I don't see any reason
to say it's wrong. But it can get very confusing, especially with words
that can be interpreted as either nouns or verbs, and I'd certainly try
to avoid it in casual conversation.

*This* is a prime subject for a FAQ entry. In an interview in HolQeD 3:3,
Marc Okrand clarified a lot of how {-ghach} should be used.

It apparently is supposed to be used only after another suffix. Using it
on a bare verb is a "highly marked" form; it can be understood, but it's
a pretty unusual thing to do. Okrand gives the pseudo-words "collapsation"
and "pleasureness" as examples of how strange it looks. There doesn't seem
to be any rule against using it on a verb that doesn't have a corresponding
noun form, as long as it follows a suffix.

If you're situation is similar to most serious students of tlhIngan Hol,
you'll appreciate having a spare dictionary around after your first one
begins to fall apart. :-)
