The first thing that comes to my mind when I see {yu'wI'} is a *person*
who interrogates, like when the suspect is sitting at a table while the
police investigator is pacing back and forth smoking a cigarette and
trying to get the suspect to talk...but I digress.
If the questionnaire is made of paper (as I think it was), then I'd
probably call it {yu'bogh nav} "paper which questions".
