I'd be happy to accept {y'}, {w'}, and {rgh} as special cases, where a pair
of sounds is treated as a single unit (which happens to occur only at the
end of a syllable).

There are two other "officially documented" places where {r} participates
in a consonant cluster, but they're both renditions of non-Klingon names:
{qIrq} and {pIqarD}.
