How about a relative clause:
De'qunDaq choHta'bogh nuv pong'e' ghItlhlu'
the name of the person who-has-changed-it will-be-written to
the page history
{vay'} works well for "whoever":
De'qunDaq choHta'bogh vay' pong'e' qonlu'
the name of whoever has-changed-it will-be-recorded in the
page history [using {qon} "record" for variety]
especially in an "if" clause:
choHta'chugh vay', De'qunDaq pongDaj ghItlhlu'/qonlu'
if someone has changed it, his/her name will be written to
(recorded in) the page history
Even simpler, use the nomen agentis of {choH} - *{choHwI'} "one who
alters/changes [something]":
De'qunDaq choHwI' pong ghItlhlu'/qonlu'
