I doubt there is a single "correct" answer. Everything you suggested works
with slightly different meanings. More below.

I'd use {tIqHomwIj} if you were trying to sound humble, like, "My heart,
which is less significant than an average heart".

This would literally be referring to size. {tIqwIj mach} is "my little
heart" or "my small heart".

If I were speaking to a small child and I were calling him something like
"my sweet little heart" or "my heartikins" or "my hearty-warty-woogie-pooh",
then I'd call the child {tIqoywI'}. Remember the child can use language, so
I wouldn't use {-wIj}. I'd use {-wI'}.

If you are literally talking about the muscles, chambers and valves that
pump blood around, {-oy} would be a very strange suffix to use on this noun.

I'd be more comfortable telling you what to say if I saw the literal context
into which this word was to go. Otherwise, the potential for
misinterpretation is both looming and potentially humorous.
