{qagh} is the tlhIngan Hol spelling. "Gagh" is the Paramount, English-ized 
spelling. (Just like {tlhIngan} is the correct name for the species, and 
"Klingon" is the Paramount, English-ized name.) As for the third way you 
mentioned, I doubt it is correct.
