What's in a name? (was: Re: Klingon babes(?))

I know most of this has been covered in numerous posts but can someone sum
it up in one post? What are the canon rules for names? I before 'e' except
after gh or tlh, that sort of thing. "Klingon never begins a word with a
consonant cluster", ok, good. That covers the beginning, now how about the
middle and the end. Any other DO's and DON'Ts ?
