Yes.  The spelling was provided in Okrand's HolQeD article (HQ 8.4) giving 
some deleted Klingon lines from ST5:

>    HQ v8n4p12 (referring to ST5: Scene 127;
>    HQ v8n4p13 (referring to ST5: Scene 174

These are the only examples of the spelling {qIrq} provided by Okrand himself:

James T. Kirk. I've followed his history since I was a boy. ST5 (ST5 notes)

The other instances of "Kirk" from ST5 and ST6 come from members' audio 
transcriptions of the dialog for which I've used Okrand's preferred spelling:
