They can certainly 'just start' in email -- the reader's attention is
already on the message.

If eye contact has been made, they can 'just start' in real life too.
No vocal 'handshake' is necessary.  Irrelevant noises before beginning
the conversation are considered rude when speaking Klingon.

If the person approached is not paying attention, the person wishing to
begin the conversation can easily say {yIqIm!} or call him by name or
even tap him on the shoulder.  Or he can use one of the words that are
intended for the purpose:  {'eH} or {Su'} or {SuH/SSS}.
