This is merely the interrogative form of of:

   Sub qoH ghap ghaH juplI''e'.
   Your friend is (either) a hero or a fool.

We have examples:

   HIq qIj reghuluS 'Iw HIq ghap jab.
   They serve Black Ale or Regulan bloodwine. CK

   naQ megh'an 'er'In ghap yI'uch.
   naQ 'er'In megh'an ghap yI'uch.
   Grasp either end of the stick.
   ("grasp the end or the other end of the stick") (HQ 12.2)

I understand your hesitation, but the second one is normal (though I think 
the first is a little stronger rhetorically).
