Never used in canon.

In the table of ranks given on KGT p. 52, an {'aj} "admiral" outranks a 
{Sa'} "general" but AFAIK, we've never seen nor heard of a Klingon admiral, 
although there have been Klingon generals (e.g. Martok (TSN & DS9), Koord 
(ST5) and Chank, Khmarr and Grokh (ST6).  (Have there been any Klingon 
admirals in  ENTERPRISE yet?)

If this is not an accident, it may be that {'aj} is not currently used in 
the "modern" Klingon Defense Forces -- like "commodore" in the modern US Navy.

As Okrand explains in KGT:

   The words used as titles to denote the rank ({patlh}) within the Klingon
   Defense Force have caused a certain amount of confusion, largely because
   the system does not neatly map onto that used by Starfleet.  [KGT 52]
